Our ACL rehabilitation pathway
Progression through each phase of ACL rehabilitation is criteria-led and supported by regular monitoring and objective testing. Strength, power and capacity markers need to be met before progressing to the next stage.
Crutches and a knee brace as needed, gentle range-of-motion work, and waking the quad back up while the graft is most vulnerable. The goal here is calm, controlled healing — not pushing through pain.
Protect
Settle swelling and irritability, restore full extension early, activate the quads and regain a normal gait pattern.
Early phaseRebuild
Progressive loading to rebuild strength and control beginning to close the strength deficit gap. Reintroduce running once markers are hit.
Early-mid phaseRetrain
More strength! bring in jumping, cutting, and landing mechanics rebuilt under load, with objective hop and strength testing guiding progression.
Late-mid phaseReturn
Work towards return to sport testing against symmetry and confidence criteria, more field-based sessions progressing towards a graded reintroduction to full training and match play.
End stageObjective ACL testing data guiding return-to-sport decisions
Objective testing is an important part of ACL rehabilitation. We use force plates and dynamometry (VALD ForceDecks and Tindeq handheld dynamometers) to measure strength and performance, helping to guide rehabilitation progression and return-to-sport decisions.
-
Strength
Isometric strength testing
Hand-held dynamometry and force plate testing are utilised to quantify strength deficits of quads, hamstrings, glutes and calves between legs, tracked phase to phase.
-
Plyometrics
Hop, jump and landing testing
Double leg, single leg, horizontal and vertical hop testing using force plate analysis, benchmarked against the uninjured leg.
-
Capacity
Essential endurance strength
Specific evidence-based capacity testing for key muscle groups required to return to higher levels of activity.
-
Readiness
Psychological readiness to return
Confidence and reducing fear of re-injury matter as much as strength and power. Periodically screened through rehab and maximised for a confident return to sport.
Athletes and active people rebuilding after ACL injury
Whether you're pre-op and wanting to plan ahead or months into a rehab that's stalled, our programming aims to get you back to the sports you enjoy, stronger and more confident than before your injury.
What ACL patients have said
"After two surgeries to repair my meniscus and then my ACL, working with Justin really helped me regain confidence in my knee again. He set me small goals and targets to hit which kept me motivated and focused on my rehab, and helped me from feelinng down with everything."
"I found the testing we did so useful and made me trust my knee again. It felt much more genuine seeing my progress in numbers and Justin helped explain what they meant and related it back to playing netball."
"A big impact from my injury what how it affected me psychologically. I found when working with Justin, he seemed to understant me and actually designed my rehab around getting strong again but also to help with a few things which I lost confidence with, at the same time."
ACL rehabilitation FAQs
Do I need a referral for ACL physiotherapy?+
No — you can book directly, whether you're pre-op, post-op, or unsure whether surgery is the right route at all.
How soon after surgery should I start physio?+
There is essential things to target in the first 1-2 weeks post-op. Physio can start as soon as you can toelrate it
Does graft type (hamstring, patellar tendon, quad tendon) change the timeline?+
Each graft type has its benefits and issues throghout rehab and require separate attention but graft choice does not typically affect duration of rehab or return to sport timelines.
When will I be able to return to sport?+
This is specific to each patient case. Generally you should plan for 12 months of rehab post-op but if all return to sport testing criteria are met before this, earlier return to sport can be discussed.
Can you help before I've had surgery?+
Yes, we know that a period of prehabilitation improves post-op outcomes. It can also serve as a good opportunity to gain baseline test scores from your uninjured leg to use as a comparator later on. It's is also a good time to discuss surgery, what the different phases of rehab involve and how to plan for this.
Do you work with insurance/private medical cover?+
Yes, currently we are registered with BUPA, AXA, AVIVA and WPA
